- W2896862120 abstract "Developmental venous anomalies (DVA) are the most common congenital vascular brain malformation. A disequilibrium in development of the pial and subependymal venous systems underlies this anomaly. DVAs typically remain asymptomatic in contrast to arteriovenous and cavernous malformations, who can present with seizures, focal neurological deficits, and hemorrhage. Sonographically, fetal DVA may mimic hemorrhagic, ischemic, infectious, or neoplastic lesions. Our 1st case was a 24-week fetus with a left periventricular non-homogenous echogenic lesion, whose diagnosis was made only on autopsy. During two weeks follow-up, the lesion showed no progressive changes. The CMA, TORCH tests, and fetal brain MRI were normal. In view of the parenchymal brain lesion of undetermined origin, the parents requested termination of pregnancy. Histological findings were typical of DVA. On retrospective review of the fetal sonographic studies, a prominent subependymal vein draining the affected parenchyma was seen on the high definition (HD) flow imaging. Following the 1st case, we have diagnosed 2 additional fetuses with cerebellar DVAs. In both cases a unilateral cerebellar echogenic lesion was detected at the 23rd week scan; the lesions slowly progressed in size during gestation w/o cystic or calcified components; the cerebellar shape and folia were normal; and a prominent parenchymal collecting vein was demonstrated by the HD flow imaging, running from the lesion toward the dural sinuses. The 32-week fetal brain MRI showed no parenchymal abnormality on the T2W, while the SWI depicted a hypointense signal, which corresponded with the collecting veins. The babies' neurologic exam was normal. Specific sonographic features of fetal DVA are described here for the first time. DVA should be considered in a fetus with a persistent, echogenic brain lesion w/o clastic or dysmorphic changes, confirmed by demonstration of a collecting vein draining the lesion and normal brain anatomy on MRI." @default.
