Moreover, MEDLINE and RSCI databases regarding surgical treatment of pancreatic diseases were assessed.There were 456 pancreatectomies. Postoperative complications arose in 176 (38.6%) patients, 11 patients died (2.4%). According to world data, mortality after pancreatectomy reaches 10%. Only creation of specialized centers is proven way to improve the outcomes.Current medical assistance for pancreatic disease may be only achieved in specialized centers with large number of various pancreatic procedures. The organization of such centers is required throughout the country and certain accreditation criteria should be developed for this purpose. Targeted routing of patients to specialized pancreatology centers will be able to reduce incidence of diagnostic, tactical and technical errors.Цель исследования - обосновать целесообразность создания специализированных клинических центров для оказания медицинской помощи пациентам с заболеваниями поджелудочной железы. Материал и методы. Проанализированы данные литературы в сети Medline и РИНЦ, касающиеся вопросов организации и тактики хирургического лечения заболеваний поджелудочной железы, а также результаты работы отделения абдоминальной хирургии Национального медицинского исследовательского центра хирургии им. А.В. Вишневского за последние 5 лет. Результаты. Выполнено 456 резекционных вмешательств на поджелудочной железе. Осложнения в послеоперационном периоде возникли у 176 (38,6%) больных, умерли 11 (2,4%) больных. По мировым данным, летальность после резекционных операций на поджелудочной железе достигает 10%. Единственно доказанный путь ее снижения - создание специализированных центров. Заключение. Оказание помощи больным с заболеваниями поджелудочной железы на современном уровне возможно только в специализированных центрах, где выполняется большое количество операций. Организация таких центров требуется в масштабе всей страны, для чего необходима разработка критериев их аккредитации. Целенаправленная маршрутизация больных в центры хирургической панкреатологии позволит сократить количество диагностических, тактических и технических ошибок." @default.
