- W2896940929 abstract "History of traumatic brain injury (TBI) is highly prevalent in older veterans. TBI is associated with risk of cognitive impairment, but the underlying pathology is not well understood. We conducted a study of remote TBI and Alzheimer's disease (AD) among older veterans living in retirement communities; 155 veterans from the Armed Forces Retirement Home, Washington, DC, and the Veterans Home of California-Yountville. Veterans provided a blood sample, completed a medical and psychiatric history and neuropsychological testing. We compared 4 groups: Control (n=57), AD (n=33), TBI-no cognitive impairment (TBI-noCI; n=30), and TBI-cognitive impairment (TBI-CI; n=35). TBI diagnosis was determined by the Ohio State University TBI Questionnaire. In those with TBI, CI was defined as a cognitive composite score > 1 SD below normative values or medical record dementia diagnosis or dementia medication. Blood plasma was analyzed across the four groups using the Quanterix Simoa Human Neurology 4-Plex, which measures neurofilament light (NFL), total tau, glial fibrillary acidic protein (GFAP), and ubiquitin carboxyl-terminal hydrolase (UCHL1). Veterans were, on average, 79 years old, mostly male, and had completed 15 years of education. The Table shows group differences on demographics and medical and psychiatric history. Within the participants with TBI, 78% reported loss of consciousness, and 35% had a history of moderate/severe TBI. Mean interval from first TBI to study visit was 51 years, and 88% of veterans had remote TBI (>2 years before visit). In preliminary analyses, none of the groups differed significantly from each other in total tau, NFL, or UCHL1 concentrations. GFAP concentrations were elevated in veterans with AD compared to the other three groups. Further analyses of blood-based biomarkers including amyloid beta and exosomal phospho-tau may help elucidate differences between these TBI and cognitive groups. Our preliminary results suggest that veterans with remote TBI, including those with versus without cognitive impairment, may not differ from veterans without TBI on blood plasma levels of proteins associated with acute TBI. Further investigation of other markers may help determine the underlying pathology of remote TBI and related CI." @default.
