- W2896957195 abstract "Brain regions exhibit different changes in Alzheimer's disease (AD). Ratios between opposing volumes from brain magnetic resonance imaging (MRI) can provide additional information to volumes. Brain 3D MPRAGE MRI were segmented into 44 regions using FreeSurfer v5.3 in 98 participants. Their size in absolute volumes and relative to the whole brain volume (normalized) were compared between 59 AD patients with Mini-Mental State Examination (MMSE) score of 21±4 points and 39 age- and sex-matched normal controls (NC) (MMSE 29±1 points). Volumes of the most atrophied parts were related to the opposing volumes of the most enlarged parts as ratios. The most atrophic structures in AD were both hippocampi (relative or absolute Cohen's effect size d between -0.79 and -0.90 (p < 0.0001). By contrast, the greatest enlargements in AD were both lateral ventricles and their inferior parts (d between 0.50 and 0.68) (p < 0.02). The best ratio for both sides with higher effect sizes was the hippocampal proportion calculated as the hippocampus / (the hippocampus + inferior lateral ventricle) (left d= - 1.00, right d = - 0.85). Its optimal cut-off of 74 % yielded sensitivity of 78 % and specificity of 76 % on the left and sensitivity of 73 % and specificity of 82 % on the right. This ratio was not influenced by age or gender in the NC group. The hippocampal proportion relative to the inferior horn of the lateral ventricle is another measure to evaluate the degree of hippocampal atrophy on brain MRI in percentages.
