- W2896971083 abstract "Arterial stiffness increases with age and with higher numbers of cardiovascular risk factors. We have previously reported in the Ginkgo Evaluation of Memory Study (GEMS) that higher pulse wave velocity (PWV), i.e. greater arterial stiffness, was associated with higher amyloid deposition in the brain. The purpose of this study was to examine whether baseline systemic arterial stiffness and cerebral β-amyloid (Aβ) deposition are associated with incident dementia in non-demented individuals age 80+. We studied a dementia-free cohort of 88 participants, mean age 83. In 2010, all participants completed MRI of the brain and PET imaging using Pittsburgh Compound B (PiB: a marker of amyloid plaques in human brain) and FDG, and were followed until 2017. We also measured resting blood pressure (BP), mean arterial pressure (MAP), and arterial stiffness by PWV in multiple vascular beds (brachial-ankle (baPWV), heart-femoral hfPWV (hfPWV), carotid-femoral PWV (cfPWV), and heart-brachial PWV (hbPWV), using a noninvasive and automated waveform analyzer. By 2017, of the 88 participants, 38% were demented, 34% mild cognitive impairment (MCI), and 18% remained cognitively normal (CN). Age-adjusted PWV was significantly correlated with PiB score and negatively with hippocampal volume and with FDG-PET. Age-adjusted PWV in the different vascular beds was higher for participants who became demented than for those who remained CN (p<0.01 for all sites). Age-adjusted incidence of dementia was 37 (8-195)/1000 person-years (PY) for the lowest quartile of cfPWV as compared to 131 (49-370), for the highest quartile. Results were similar comparing other vascular beds. In Cox proportional hazards models, hazards ratios (HR) for PWV were all elevated. In the models that included age, amyloid deposition, and hippocampal volume, the strongest association was for hfPWV (HR: 8.8, CI: 1.06 – 70.5). Age, hippocampal volume, and amyloid deposition were all significant predictors in this model. Arterial stiffness as measured by PWV in different vascular beds is a predictor of both amyloid deposition in the brain and the risk of dementia in elderly individuals after 7 years of follow up.
