- W2897087015 abstract "While physiological urine output is accepted to vary according to weight in children and adults, adjustment for fetal weight has not been recognised as useful when assessing amniotic fluid volume. However, previous analyses have examined pregnancies with predominantly normal amniotic fluid levels. The value of adjustment for fetal weight in pregnancies with polyhydramnios has not yet been determined. This retrospective audit was conducted at a tertiary fetal medicine unit in London, UK. Cases were identified from the unit's fetal medicine database. All cases referred with polyhydramnios (AFI >95th centile) between January 2015 and January 2016 were included. In 36/120 cases fetal abnormality was identified following fetal medicine assessment. Mean amniotic fluid index (AFI) did not differ between these cases and those with no abnormality, nor was there a significant difference in the incidence of fetal abnormality when cases were stratified according to AFI. Significant differences were observed in the gestation of referral, with abnormal cases having an earlier mean gestation (28.9 vs. 32.3 weeks, p=<0.001), and lower estimated fetal weight (1549g vs. 2194g, p=<0.001). However, estimated weight percentile did not show a difference between the groups (46 vs. 49, p=0.29). When amniotic fluid volume was adjusted for estimated fetal weight (AFI in cm/kg), significant variation in the incidence of fetal abnormality was observed (p=0.01) (figure 1). Normalisation for fetal weight may be a valuable discriminator in determining which cases of polyhydramnios require further investigation and may facilitate counselling. In particular, an AFI cm/kg of >40 was associated with fetal abnormality in all cases. Further prospective examination of this novel measure is indicated.
