Imprecise diagnostic methods and too much trust in their outcome by clinicians may cause part of these differences. Diagnostic thresholds as used in interviews or scales, yield limits to sensitivity and specificity, leading to both false-negatives and -positives, respectively, alongside correct negatives and positives. There is a reciprocal and inverted effect: higher sensitivity yields lower specificity. The accuracy of a diagnosis can be viewed as the increase in probability for a “true” diagnosis [Diagnostic Likelihood Ratio (DLR)] when the diagnostic threshold is passed and calculated by a formula: DLR = sensitivity/1-specificity. The DLR for a diagnostic step can be added to the initial probability of a specific diagnosis (eg, ADHD), using a “nomogram” to obtain the increased likelihood of a “true” diagnosis using Bayesian logic." @default.
