- W2897327280 abstract "Pseudobulbar behavior is frequent across a wide range of neurocognitive disorders. Case Presentation AG is a 48 year old college educated, independent living Ethiopian American with a history of Bipolar I with Schizoaffective disorder. The patient was admitted to Georgetown University Hospital in July 2016 with severe viral encephalitis. Patient had a 6 week ICU course with ventilator support. On ICU discharge the patient was severely agitated, violent, cognitively impaired, speech impaired and had a combined choreo-athetoid and Parkinsonian movement disorder with frequent falls. He was eventually diagnosed with post-encephalitic Cerebellar-Cognitive-Affective Syndrome (Schmahmann Syndrome). Despite multiple attempts at behavioral stabilization with psycho-pharmacotherapy, including high dose neuroleptics, the patient remained hospitalized in physical restraints and 24 hour/day 1:1 direct supervision when we were initially consulted in April 2017. AG was evaluated and transferred to our inpatient care in Indiana. The patient was evidencing repetitive, stereotyped outbursts without any external referents. This was consistent with Pseudo-Bulbar Behavior Syndrome. AG was started on dextromethorphan/quinidine (Nuedexta). Within seven days he demonstrated marked reduction in behavioral dysfunction. Within twelve days of initiation of Pseudo-Bulbar treatment, AG experienced complete resolution of agitation, violence and outbursts that had been persistent since ICU discharge in August 2016. This case is consistent with a complex pattern of Pseudo-Bulbar behaviors that we are diagnosing and treating in a wide variety of neurocognitive disorders, including Alzheimer's, Lewy body, Parkinson's and vascular dementias. Dextromethorphan/quinidine is specific exclusively for Pseudo-Bulbar disorders and therefore clinical response to this therapy is pathognomonic for this disorder." @default.
