- W2897345283 abstract "Lower concentrations of IGF-1 have been related to an increased risk of Alzheimer's disease in older adults, but associations between IGF-1, or its binding protein IGFBP-3, and early MRI markers of brain aging in younger populations have not been established. This study investigated the associations of serum IGF-1 and IGFBP-3 with MRI markers of brain aging in middle-aged adults in two epidemiologic cohorts. We included 1852 participants (mean age 46±9, 54% women) from the Framingham Study third generation cohort and 674 participants (mean age 50±13, 58% women) from the Study of Health in Pomerania with circulating biomarker measurements and MRI. Serum IGF-1 and IGFBP-3 concentrations were measured using standard immunoassays. Outcomes included MRI measurements of total brain, cortical gray matter, white matter, ventricular, and white matter hyperintensity volumes. Each cohort related IGF-1 or IGFBP-3 concentrations to MRI outcomes using multivariable regression models adjusting for potential confounders, including cardiovascular risk factors. We additionally tested for interactions with blood concentrations of C-reactive protein (CRP) dichotomized by the 75th percentile, a marker of systemic inflammation. Finally, we meta-analyzed summary statistics from each cohort using random effects models. Meta-analysis results revealed that higher IGF-1 concentrations were associated with larger hippocampal volumes (estimate [β]±standard error [SE], 0.07±0.03, p=0.005). We found no associations for IGFBP-3 in the overall sample. In both cohorts, we observed higher IGF-1 concentrations in individuals with lower CRP compared to those with higher CRP concentrations (p<0.001); but no IGFBP-3 differences by CRP group (p<0.073). Meta-analysis results stratified by CRP concentrations revealed associations between higher IGF-1 and larger hippocampal volumes (0.100±0.031, p=0.001), and higher IGFBP-3 and larger cortical gray matter volumes (0.045±0.021, p=0.030) in individuals with low CRP concentrations. Circulating IGF-1 and IGFBP-3 concentrations are associated with early markers of brain aging in middle-aged adults from two population-based samples. This association occurs predominantly in individuals who do not have co-occurring systemic inflammation. Our findings suggest that IGF-1 related signaling is implicated in brain health as early as midlife. Further studies relating IGF-1 and IGFBP-3 concentrations to MRI markers of brain aging are warranted in other middle-aged populations." @default.
