- W2897445907 abstract "There is growing attention on the vascular component of neurodegenerative disease. Arterial transit time (ATT), is a physiological parameter reflecting haemodynamic function. There is conflicting literature on the association of prolonged ATT with Alzheimer's disease (AD)1,2, and to the best of our knowledge, no investigations of ATT in mild cognitive impairment (MCI). A new and reliable method of estimating ATT from arterial spin labelling (ASL) images is to calculate the spatial variation of grey matter ASL signal across the brain (spatial-CoV). Spatial-CoV has been shown to correlate with age and sex, making it a candidate measure of vascular health3. Here, we investigated how spatial-CoV varies in MCI subtypes and early AD, and hypothesised increased spatial-CoV would be associated with reduced cognitive abilities. 75 subjects (controls [C; N=17], subjective cognitive decline [SCD; N=19], amnestic MCI [aMCI; N=18], multi-domain MCI [mMCI; N=14] and early Alzheimer's disease [AD; N=7]) enrolled in the longitudinal Brain Research New Zealand, Dementia Prevention Research Clinic were scanned on a Siemens Skyra 3T, with a 3D pCASL sequence, whole brain coverage, post-labeling delay=1.8s. Spatial-CoV of partial volume corrected, grey matter perfusion was calculated with ExploreASL4. Subjects were classified following a multi-disciplinary assessment, including the Addenbrooke's Cognitive Examination-III (ACE-III), detailed neuropsychology and neuroimaging. Figure 1 shows spatial-CoV measured in the 5 groups. An ANCOVA showed a main effect of group on spatial-CoV (age and gender as covariates; F(4,68) = 6.43, p=.0002). In addition, a linear contrast showed spatial-CoV increased with symptom severity (i.e. AD>mMCI>aMCI>SCD>C; p=.00001). Bonferroni corrected post-hoc comparisons showed both control and SCD groups had lower spatial-CoV than both mMCI and AD groups, but not the aMCI group. Figure 2 shows spatial-CoV plotted against ACE-III for all subjects, and was negatively correlated (Spearman's rho=-0.30, p=0.01, after correction for age and sex). These results show this new imaging metric may be useful to study vascular health, tracking progression to MCI and onto early AD.
