- W2897460862 abstract "Cognitive reserve appears to delay the onset of dementia, but once a critical threshold is reached the progression of dementia in terms of effects on cognitive decline is more rapid which in turn leads to fewer years before mortality. Here we evaluate the role of reserve in transitions between cognitive states and examine replicability of results using data from 6 longitudinal cohorts of older adults. Multi-state modeling (MSM) allows for a better understanding of the role of risk factors in transitions between cognitive states. We fitted MSM models to cognitive scores from 6 international cohorts (OCTO and H70(Sweden), LASA (Netherlands), LBC1921 (Scotland), MAP (US), Whitehall II (UK)), where cognitive states were defined using MMSE cut off points. Transitions between cognitive states were adjusted for age, education and socioeconomic status. In OCTO-Twin, LASA, Whitehall & MAP, high/medium vs. low level of education was associated with lower risk of transitioning from no cognitive impairment to MCI. In OCTO Twin, medium vs. low level of education was associated with higher risk of transitioning from no cognitive impairment to mortality. In LASA, H70, and MAP, higher level of SES was associated with a decreased likelihood of transitioning from no cognitive impairment to MCI. In LASA, higher level of SES was associated with a decreased likelihood of transitioning from MCI to severe cognitive impairment. In all studies, Individuals with lower SES and education level had shorter non-impaired life expectancies. Education may be more protective against cognitive decline in the earlier stages of the disease progression and in non-pathologic cognitive aging but less so for the later stages The inclusion of several studies allowed replication of the same analysis and presentation of results across studies, allowing evaluation of the generalizability of our results. Validation of results using clinical diagnosis to define states is ongoing in two longitudinal studies of older adults." @default.
