- W2897478545 abstract "Cerebral small vessel disease (SVD) frequently coexists with amyloid pathology in Alzheimer's disease (AD), and these pathological processes may interact. White matter (WM) lesions are hallmarks of SVD. They are usually assessed by WM hyperintensities on T2-weighted MRI images, but WM hypointensities on T1-weighted images may be an equally good measure of SVD. The aim of this study was to explore whether WM hypointensity volumes (WMHo) are related to AD predementia markers and APOE. Cases and controls, aged 40-80, were included from the DDI longitudinal multi-site study (n=343, age=63.1, m/f=157/186). Participants underwent clinical and cognitive assessment (n=339), MRI (n=343), APOE genotyping (n=326) and lumbar puncture (n=307). Cases were staged as Subjective Cognitive Decline (SCD) or Mild Cognitive Impairment (MCI), and further stratified by Ab1-42 pathology (Ab+/-), using an amyloid-PET verified CSF cutoff. Cognitively normal Ab- controls were classified as Healthy Controls (HC). APOE-e4 carriers were defined as APOEe4 positive (APOEe4+). WMHo was determined using FreeSurfer. Partial correlation analysis was done between WMHo (normalized against intracranial volume) and CSF Ab, stratified by APOE status. The interaction between WMHo and APOE (on CSF Ab) was tested in a linear regression model. WMHo was log-transformed and compared between groups by linear regression. All analyses were adjusted for age. There was a significant correlation between WMHo and CSF Ab in APOEe4+ subjects (n=134) (r=-0.28, p=0.001), but not in APOEe4- subjects (n=164) or the overall cohort (n=307). The interaction between WMHo and APOE was significant (n=289, p<0.001). The SCD-Ab+ group (n=19) had significantly higher WMHo than SCD-Ab- (n=105) (p=0.042) and HC (n=56) (p=0.026), and also the MCI-Ab+ group (n=41) had significantly higher WMHo than MCI-Ab- (n=66) (p=0.017) and HC (p=0.004). There was no significant difference between the Ab+ SCD and MCI groups. SCD-Ab+ cases had higher levels of WMHo than both healthy controls and SCD-Ab-. There was a correlation between WMHo and CSF Ab levels in APOEe4+, and a significant interaction between WMHo and APOE on CSF Ab levels. These findings indicate an interplay between APOE-e4 and SVD, and further implicate SVD as a feature of early predementia AD." @default.
