Follow-up rates were high. Major disability was defined as moderate or severe cerebral palsy, blindness, deafness, or a general intelligence Z score less than −2 relative to term-born controls. Increasing gestational age at birth and increasing postnatal age were associated with later decreased mortality and increased survival without major disability, but did not affect survival with major disability. Four neonatal morbidities were associated with disability: grade 3 or 4 intracranial haemorrhage, cystic periventricular leukomalacia, postnatal steroid administration for lung disease, and any surgery. Of the 499 children who survived to discharge with 8-year outcome data, 241 (48%) had no major postnatal events and had the highest probability of survival without major disability (0·93 [95% CI 0·89–0·96], in contrast to 0·31 [0·11–0·59] in survivors who had three or more such events).
