- W2897540062 abstract "This study aims to determine the prevalence and accuracy of using TVS and 3D-UTS in the diagnosis of MA. This is a retrospective descriptive study of women with ultrasound findings of MA who underwent surgery at the University of the Philippines, Philippine General Hospital from January 2013 to December 2016. The Department of Obstetrics and Gynecology master list of admissions with a diagnosis of possible MA in their charts and ultrasound reports were reviewed to determine age, gravidity/parity, and presenting symptoms. MA prevalence rate and diagnostic accuracy estimates: sensitivity, specificity, positive and negative predictive values of TVS and 3D UTS and comparison with surgical findings as gold standard in the diagnosis of MA were determined. The local prevalence rate (PR) of MA was 1.2%. About 81% were nulligravids with an average age of 20. The most common presentations were cyclic pelvic pain (46.9%), infertility (12.5%), bleeding (9.4%), and dysmenorrhea (9.4%). The most common MA was transverse vaginal septum (34%), uterine didelphys (31.2%), and septate uterus (21.9%). All 32 cases had TVS and only 7 cases had 3D UTS. Both the 3D UTS were similar with TVS in 78%. Uterine didelphys (p=0.044) and transverse septum (p=0.001) ultrasound findings were significantly associated with intra-operative findings. TVS and 3D UTS had variable sensitivity, specificity, positive and negative predictive values depending on the type of MA. Both imaging methods had high specificity for uterine didelphys and transverse vaginal septum but weak sensitivity. The local MA prevalence rate is low but reflects literature rates. A prospective study with longer study period can improve diagnostic accuracy for MA. Factors affecting accuracy include operator skills, non-standardisation of classification systems, non-uniform diagnostic modalities, and varying populations of women.
