- W2897545810 abstract "Reproductive window (RW; age of menarche to menopause) has been associated with risk for Alzheimer's disease (AD), but modifying factors have not been well-studied. We investigated the relationship between RW and AD in a population-based sample. 2575 adult women, aged 65+, without dementia at baseline were followed up to 12 years. Dementia screenings and assessments were conducted in four triennial waves to identify those with dementia and non-cases (screened or evaluated negative for cognitive impairment). Dementia determination was based on a clinical assessment, and diagnosis of AD was made using DSM-III-R and NINCDS-ADRDA criteria. A women's health questionnaire (WHQ) was administered that queried reproductive history and use of hormone replacement therapy (HRT). Cox proportional hazards regression examined reproductive window (in tertiles; RWT) and hazard of AD, controlling for age and testing interactions (hysterectomy, APOE-E4 and RWT). 1404 women [mean(SD) age = 73.86(6.44)] completed the WHQ and were included in the analyses. In a model controlling for age and APOE-E4, only women in the middle tertile had higher risk of AD (HR = 1.90, 95% CI = 1.34-2.71) compared to those in the highest tertile. However, there was a significant interaction between RWT, hysterectomy and APOE-E4 (p=.017). In the lowest RWT, APOE-E4 carriers with hysterectomy exhibited lower risk of AD (HR = 0.23; 95% CI = 0.06-0.79) compared to those without hysterectomy. Sixty-two percent of those in the lowest RWT had a history of hysterectomy (cf. 39.6% and 34.4% in the middle and highest tertiles, respectively; X2(2) = 83.19, p <.001), and hysterectomy was associated with HRT in all RWT (66-80% use with hysterectomy and 43–51.5% without). HRT was initiated at a lower age in women in the lowest RWT (F(2,786) = 108.58, p<.001) with mean differences of 7.6 to 11 years. In a community-based sample of older adults, a shorter reproductive window was associated with higher risk of AD (moderated by APOE-E4 and hysterectomy). Hysterectomies were associated with HRT use and possibly reduction in AD risk. Future work will further examine modifying factors between reproductive history and AD risk." @default.
