- W2897597878 abstract "The subtle cognitive decline preceding the cognitive impairment onset [1] can be self-perceived, referred to as subjective cognitive decline (SCD), or go unrecognised [2]. In some of the latter cases a close relative may perceive it. The cognitive and imaging characteristics of this group that lacks awareness of cognitive decline that is perceived by others are largely unknown. We aimed at studying the frequency of cognitively normal subjects without self-awareness of cognitive decline that is reported by an informant and to explore their cognitive and structural neuroimaging correlates. 2640 cognitively healthy participants from the ALFA population-based study underwent clinical, cognitive and structural MRI explorations (subset of 530). Subjects were categorized as unaware decliners (UD; decline reported by the informant only) or as SCD based on their SCD-Q answers [3]. Cognitively normal participants that did not fulfill these categories were used as controls. Memory was assessed using the Memory Binding Test and executive functions using WAIS-IV subtests. Linear models adjusting for confounders (age, sex, education and mood) were used to test group differences on cognition (p<0.05) and voxel-wise GM volumes (punc<0.001; K>100). UD frequency was 6.6% while SCD was 21.5%. As compared to controls, UD and SCD showed lower performance in memory (p<0.05). No differences in anxiety and depression were observed between controls and UD, while SCD did (p<0.01). UD outperformed SCD in delayed free recall (p=0.02) and visual reasoning (p=0.02). Medial frontal and insular increments of GM volumes were observed in UD with respect to controls. Presence of informant report of decline was associated to lower hippocampal GM volumes that were related to memory performance in UD subjects only (rho=0.46, p=0.002). UDs present worse memory performance than controls which correlated with hippocampal GM volume. These results support the usefulness of informant-reported cognitive decline in cognitively normal subjects and that participants unaware of their cognitive decline present different cognitive and brain morphological features than controls.
