• W2897598636 abstract "Informal family caregivers provide a majority of care for persons with dementia (PwD). It is necessary to provide informal caregivers with effective support and skills. However, little is known about the affordability, value, and perceived need by caregivers of proven skills interventions. During baseline interviews for the Dementia Behavior Study, caregivers were asked prior to randomized treatment assignment how much they would be willing to pay (WTP) per session for an 8-session program that lasts 3 months, was delivered by healthcare professionals in their home, and provided them with skills to address problem behaviors of PwD and stress reduction techniques (response options: $0/session, $25/session, $50/session, $75/session, $100/session, $125/session, $150/session, $175/session, $200/session, or other). We stratified the sample into those WTP>$0, those WTP $0, and those who refused to provide a WTP. First, we calculated mean WTP. Then, we used a two-part model (1: logistic regression to model the probability a caregiver was WTP>$0, 2: generalized linear model with log link and Gamma distribution to estimate WTP conditional on WTP>$0), controlling for demographic characteristics, to examine associations between WTP, the PwD's cognition (measured using the Mini-Mental State Examination), function (measured using the Caregiver Assessment of Function and Upset scale), behavioral symptoms (measured using Neuropsychiatric Inventory - Clinician), and time spent providing PwD assistance with daily activities. We predicted the joint marginal effects which combined the effects from the first and second parts of the model. Of 250 dyads enrolled, 226 (90%) had complete data and were included in our analyses. Of 226, 129 (57%) were WTP>$0, 77 (34%) were WTP $0, and 20 (9%) refused to provide a WTP value. Mean adjusted WTP was $34.48 (95% CI: $27.13, $41.84) per session. The clinical features were not significantly associated with WTP. One-additional hour providing PwD assistance was associated with a $1.46 (95% CI: $0.17, $2.74) increase in WTP per session. Most informal dementia caregivers valued an in-home skills intervention. More time spent providing care with daily activities is associated with caregivers WTP more for skill programs. Policymakers need to consider payment models for nondrug interventions." @default.
