- W2897625965 abstract "Gait disturbance is the most striking symptom of patients with idiopathic normal pressure hydrocephalus(iNPH). An increased stride-to-stride variability can predict the risk of falls in community living elderly people and in patients with iNPH. Eighteen patients with iNPH (8 men and 10 women, mean age 72.61 ±6.86 years, range 60–84 years) were included in the study. Gait disturbances of patients with iNPH were evaluated before and after CSF drainage(CSFTT). Gait was analyzed with a computer-based, 5.8-m-long pressure-sensitive carpet system (GAITRite, CIR System, Havertown, PA) with a sampling rate of 120 Hz. Relevant temporal and spatial gait cycle parameters were recorded while patients walked 4 times. The data used in the analysis were obtained by averaging the four times. After the CSFTT, walking velocity was improved(t= -3.11, p <.01), the frequency of steps was reduced (t= 2.85, p< .05), and the ambulation time was shortened(t= 3.13, p <.01). These results are considered to be influenced by several parameters as follows: increases of the step length(pre:33.57±3.24 vs. post: 37.78±3.51, p< .01), stride length(67.52±6.50 vs. 76.30±7.09, p< .01) and stride velocity(60.33±6.08 vs. 70.62±6.90, p< .01), reductions of toe degrees(14.30±1.95 vs. 13.38±1.98, p< .05). Interestingly, the CSFTT improves not only the single gait parameters but also the variability of gait. It showed that the standard deviations of step time, stride length, stride time, swing time, stance time were significantly decreased, respectively p< .05, p< .05, p= .062, p< .05, p< .05. Especially, The reduction of stride-to-stride variabilities known as a sensitive marker of gait stability were deeply impressive. After the CSFTT, Coefficient of Variability(CoV) of stride time(9.66±2.38 vs. 6.53±1.01, p= .06) and stride length (14.08±2.42 vs. 9.56±1.07, p< .05) were significantly decreased. Stride-to-stride variability, specifically stride time variability(STV), is a sensitive marker of gait stability. It has been suggested that it reflects one of the final most highly integrated CNS pathways of gait. This study showed that after the CSF drainage, the patients with iNPH were improved the stride-to-stride variability as well as general gait parameters." @default.
