- W2897626372 abstract "Napping, both intentional and not, is very common in the elderly. Growing evidence suggests a close link between napping and adverse health outcomes in older adults. However, few studies have examined napping objectively, and little is known about the effects of napping on neurodegeneration. We aimed to study the association between actigraphy-measured napping and risk of developing dementia in community-dwelling older men. We studied 2751 older men (mean age 76 years) without significant cognitive impairment at baseline. Naps were defined as ≥5 consecutive minutes of inactivity by actigraphy, and napping duration was the accumulated time of naps outside the main sleep period. Dementia was defined as having a physician diagnosis of dementia, Alzheimer's medication use, or a ≥1.5 SD above the mean change in global cognition test score. We used multivariable logistic regression to examine the association between baseline napping duration and risk of developing dementia over 12 years. We also excluded cases identified within the first two years after baseline to better understand the direction of the association. Overall, 1061 men napped for less than 30min, 795 napped for 30-59min, 642 napped for 60-119min, and 253 napped for at least 120min per day. We identified 522 incident dementia cases over 12 years. After adjustment for age, education, body mass index, smoking, physical activity, depression, comorbidities, sleep medication use and baseline global cognition, men with a napping duration of ≥120min at baseline were 52% more likely to develop dementia (OR=1.52, 95%CI: 1.04-2.34), compared to men with a napping duration of less than 30min (overall p<0.01). After excluding dementia cases identified within the first 2 years after baseline napping measures (n=157), the ORs (95%CI) were 1.17 (0.87-1.56), 1.30 (0.96-1.77) and 1.80 (1.21-2.67) for those napping for <30min, 30-59min, 60-119min, and ≥120min, respectively. Further adjustment for nighttime sleep duration, efficiency and hypoxemia did not appreciably alter the results.
