- W2897714994 abstract "Subjective Cognitive Decline (SCD) increasingly emerged as a risk condition for Alzheimer's disease (AD). However, the effect of CR on brain structure in SCD has not been described yet. The present study aimed to investigate the effect of CR on i) AD MRI proxies, ii) white matter hyperintensities (WMH) and iii) white matter (WM) microstructural integrity in older adults with SCD. 127 SCD enrolled in 2 sites underwent 3T MRI. CR was measured by the Cognitive Reserve Index (CRI; Nucci et al.,2012), a comprehensive measure of education, occupation and leisure activities. Subgroups were defined according to CRI distribution: High Reserve group (HR=32) included participants in the upper quartile (CRI>137); matched Low Reserve group (mLR=32) included participants under the median (CRI<123) matched to HR for global cognition, age, gender and site. AD MRI proxies (FreeSurfer6.0): hippocampal and lateral ventricles volumes adjusted for total intracranial volume (TIV), AD cortical signature (Dickerson et al., 2009). WMH volumes: measured using lesion segmentation tool on SPM12 and normalized as a percentage of TIV. WM integrity: voxelwise between groups differences in fractional anisotropy (FA) computed using Tract-Based Spatial Statistics (p=0.05, TFCE corrected), with TIV, age, site as covariates. HR displayed higher education (HR=18±3, LR=12±3; p<0.001) and TIV (HR=1376±160cc, mLR=1495±210cc; p=0.013). No significant between group differences were found in AD MRI proxies and WMH volumes. As to WM integrity, HR compared to mLR displayed widespread increased FA on the entire skeleton, with the highest significance in: genu and splenium of the corpus callosum, anterior corona radiata, fornix and cingulum (cingulate gyrus). Conversely, they displayed reduced FA in the body of corpus callosum, superior corona radiata, posterior limb of internal capsule and right cingulum (hippocampus). HR and mLR differed for established reserve proxies (TIV and education). In contrast to previous findings in AD patients, AD MRI proxies and WMH resulted comparable between groups. The widespread increased WM integrity found in HR compared to mLR suggests a prominent neuroprotective mechanism of CR in SCD. Moreover, the reduced WM integrity found in tracts crossing brain regions early affected by AD pathology, could explain concurrent regionally-specific compensation mechanisms." @default.
