- W2897716330 abstract "The blood brain barrier (and blood-CSF barrier) (BBB) is a vital part of the brain, offering protection by providing a physical, chemical and enzymatic barrier between the blood and neuronal environment. Dysfunction of the BBB has been associated with abnormal cerebral beta-amyloid (Aß) levels and inflammatory processes that can also have downstream effects, resulting in neurodegeneration. We set out to investigate the link between CSF/plasma albumin ratio as a metric for BBB-dysfunction and CSF p-Tau as a marker of neurodegeneration and the potential role of interleukin-6 (IL-6) and Aß therein. Ninety-seven participants with Subjective Cognitive Decline (SCD), Mild Cognitive Impairment (MCI) and Alzheimer's disease (AD) (see figure 1 for demographics) were recruited from the memory clinic of the Maastricht University Medical Center. Biochemical analysis of CSF Aß1-42 and p-Tau181p (ELISA), serum IL-6 (cytometric bead array), CSF/plasma albumin ratio (nephelometry) and APOE genotyping was done. Data was analyzed using Spearman's rank correlation analyses and robust Huber-M mediation and moderation analyses. Moderations were probed using the Johnson-Neyman (JN) procedure to determine the region of significance at p< 0.05. All analyses were corrected for age, sex and diagnosis. Demographics Partial Spearman's rank correlations revealed that only IL-6 correlated with albumin ratio (r=0.28, p=0.006). We observed no independent effects, but a synergistic effect of IL-6 and albumin ratio on Aß (Z=-2.72, p=0.006). The JN-analyses showed that the association between albumin ratio and Aß was significant for IL-6 levels ≥3.48 pg/ml. We found no direct relation between the albumin ratio and p-Tau levels (p=0.91), but mediation analyses indicated that the mediation of Aß,on the association between BBB dysfuction and p-Tau, was contingent on elevated IL-6 levels (indirect effect for IL-6: mean-1σ (β=0.026, Z=0.075, p=0.94), mean (β=1.16, Z=1.64, p=0.09), mean+1σ (β=3.51, Z=0.197, p=0.049)). There is a negative relationship between Aß and p-Tau (p<0.001). The results suggest a link between BBB-dysfunction and markers of neurodegeneration via Aß-mediated mechanisms in the presence of elevated IL-6 levels, indicating that inflammation and BBB-dysfunction, in addition to Aß are important components in the path to neurodegeneration. These results fit with autopsy studies reporting a common co-occurrence of Aß and vascular pathology in AD-type dementia .
