- W2897728830 abstract "Cardiovascular health plays an integral role in the risk of dementia. There is known sex bias in both vascular disease and dementia, with women having higher rates of vascular disease following menopause, and two-thirds of all dementia cases being women. Despite this, few reviews have focused on sex differences when considering modifiable risk factors for disease. Without the knowledge of sex-specific differences in vascular risk factors for dementia, preventative trials and therapies are constrained. This systematic review and meta-analysis aims to summarize the sex differences in known vascular risk factors for dementia; hypertension, obesity, diabetes and hypercholesterolemia. This review adhered to PRISMA and MOOSE guidelines and is submitted in PROSPERO. Medline, EMBASE and PsychINFO databases were searched for terms pertaining to hypertension, obesity, diabetes, hypercholesterolemia and mild cognitive impairment (MCI), dementia, Alzheimer's disease (AD), or cognitive decline. After duplicates, 6,112 articles were retrieved. Inclusion criteria comprised articles reporting the sex-specific risk of disease or cognitive decline. Studies were excluded if they didn't adjust for at least age, or were conducted in another disease population. Random-effects meta-analysis is underway to examine the risk of dementia and cognitive decline in males compared with females for each vascular risk factor. Study quality, heterogeneity and publication bias will be assessed using the NIH quality assessment tool, I2 statistic and funnel plots respectively. Not all studies examining the association between vascular risk factors and dementia report findings separately for men and women. Many studies examining risk factors for dementia also exclude people with cardiovascular disease at baseline, which limits our understanding of midlife vascular health and later-life disease. Other cardiovascular studies that have measured later-life cognition, and that show sex-differences are limited by selection bias in recruiting primarily cardiovascular patients at baseline, or lacking trajectories of neuropsychological assessment. Lessons from cardiovascular health research have taught us that sex differences can lead to significant variances in the diagnosis, treatment and management of chronic diseases related to vascular change. It is crucial that cohort studies explore data collaborations to better understand sex differences in modifiable risk factors for cognitive decline and dementia." @default.
