- W2897788896 abstract "Areas of white matter damage that appear hyperintense on T2-weighted magnetic resonance images are commonly referred to as white matter lesions (WML). Pathology occurring within the cerebral white matter disrupts central conducting pathways, impacting via impaired axonal function. Although WML are common in healthy aged individuals, their presence is associated with an increased risk of dementia, including Alzheimer's disease. As the white matter does not contain dendrites or synapses different mechanisms to those identified in cortically-focused dementia research are likely to be important. Age-associated WML are associated with vascular risk factors and small vessel disease, suggesting vascular insufficiency and cerebral hypoperfusion underlie lesion formation, but recent pathology studies suggest a more complex pathogenesis. Detailed immunohistological characterisation and microarray gene expression profiling of samples from the UK population-representative Cognitive Function and Ageing Study (CFAS) neuropathology cohort support a role for hypoxia/ischemia in WML formation. However, these approaches also suggest that blood brain barrier dysfunction, immune activation, mitochondrial dysfunction and glial cell injury also contribute to lesion pathogenesis. Furthermore, these pathological changes are not confined to WML, but are also a feature of the radiologically normal appearing white matter in lesional cases, suggesting that WML occur in a field-effect of abnormal white matter. Given the very limited success of current therapies, which offer limited efficacy, it is highly likely that successful treatment of dementia will require the identification and modulation of alternative mechanisms causing neuronal dysfunction and neurodegeneration." @default.
