- W2897890173 abstract "Sulci widths have been studied to characterize age related brain morphology and are relevant markers of brain atrophy. Little is known about how sulci widening alone or in combination could be an early marker of dementia risk. We aimed at identifying a sparse selection of sulci predicting risk of dementia onset in a large sample of non demented-persons at enrollment. The Memento cohort enrolled 2323 patients in 26 French memory clinics. At entry participants presented either mild cognitive impairment or subjective cognitive impairment, agreed to have a brain MRI and to be followed-up at least 5 years. Sulci widths (61/62 on right/left hemisphere) were assessed through an automated procedure from 3D-T1 brain images. At least yearly, neuropsychological and neurological evaluation are performed and dementia diagnosis is assessed by clinicians, based on DSM IV criteria and NINCDS-ADRDA for Alzheimer's disease and confirmed by an independent adjudication committee. Incident dementia risk was estimated using delayed entry Cox models. A sparse selection of sulci was performed using the Elastic Net method. The discriminative performance of the best selection was evaluated using Cross-Validated Area Under the Curve (cvAUC) and 95% confidence interval (CI) were provided. 2096 patients were included in the analysis (61% female, median age of 72 years, 55% with education level ≥ 12 years, 30% APOE ε4 carriers, 16% SCI). During a median 43 months of follow-up, 227 persons were diagnosed with dementia. We identified a subset of 22/123 folds, mainly located in temporal and frontal lobes, leading to a cvAUC of 0.81 [95%CI 0.78-0.84] (table). After addition of both the sum of the free recall of the Free and Cued Selective Reminding Tests and APOE ε4 status, the final cvAUC was 0.91 [95%CI 0.89-0.93]. A sparse selection of 22 sulci led to a prediction model with high performance to discriminate between future dementia cases and non-cases. This combination represents an objective biomarker that could be helpful to improve the selection criteria into future clinical trials, and a valuable tool for clinicians to identify people at high risk of dementia independently of their cognitive or genetic status." @default.
