- W2897894811 abstract "[18F]MK-6240 is a second-generation PET ligand for in vivo detection of neurofibrillary tangle pathology. The purpose of this analysis was to assess relationships among tau image signal, amyloid burden and cognition across a broad spectrum of people. 51 participants (mean age 66 ±10; 28F, 23M; 3 young controls, 39 cognitively unimpaired older controls of whom 33 were stable and 6 exhibited decline, 2 MCI, 7 probable AD dementia) underwent [18F]MK-6240 (70-90min), [11C]PIB (0-70 min) and MRI. The [18F]MK-6240 images were normalized with an inferior cerebellar GM reference region and warped to MNI space. Voxel-wise regression was used to examine the relationship between an amyloid burden index and MK6240 images. Pearson correlations between tau in the inferior temporal gyrus (ITG, a representative region) and neuropsychological measures were computed. 43% of the sample were amyloid positive(+). Tau+ signal (Figure 1) was seen in 13/51 participants (5/7 persons with dementia; 1/2 with MCI; 3/6 with cognitive decline; and 4/33 cognitively unimpaired). 12/13 (92%) tau+ cases were also amyloid+. Amyloid burden correlated strongly with tau signal across the whole sample (Figure 2), particularly in the medial and ventrolateral temporal lobe. In addition, breakdown of tau Braak-like staging by clinical stage and amyloid status indicated the association with amyloid and Braak stage was evident across clinical stages (Figure 3). Across the whole sample, moderate correlations between ITG tau burden and cognition were observed (AVLT total r=-.51, p<.05, Logical Memory IIA r=-.2, p NS; TrailMaking B r=.6, p<.05; and Semantic Fluency r=-.36, p<.05). Within the sample of 39 non-impaired participants, only semantic fluency was significant (r=-.46, p<.05).
