- W2897906187 abstract "As a stand-alone treatment, memory training (MT) programs for people with dementia (PWD) have been shown to be moderately efficacious. However, they can be difficult for this group of patients, as they serve as a constant reminder of cognitive deficits, which can lead to low self-esteem and a sense of hopelessness. Because of these limitations in standard MT programs, add-on therapies directly targeting hopelessness and self-esteem may enhance the efficacy of MT. 44 outpatients diagnosed with early to moderate Alzheimer's disease were randomly assigned to 4 months of (1) computer-based MT, or (2) a cognitive behavioral therapy (CBT) regimen in conjunction with the same MT (CBT+MT) designed to target negative expectations and automatic distorted schemes of hopelessness and perceptions of poor self-competency for the training exercises. Assessments of cognitive function, quality of life, depression, neuropsychiatric symptoms, treatment adherence, and perceptions of self-efficacy were conducted at intake, immediately following treatment conclusion (post-treatment), and at 1-year follow-up. The CBT+MT group showed greater self-efficacy during the training phase than the MT alone group. At post-treatment, the CBT+MT group showed better memory and less depression than the MT alone group (F[4, 34]=3.84-3.99; p<.013). Additionally, completion rates were much higher for the CBT+MT group (82%) compared to the MT alone group (69%). There was no difference in symptom presentation at post-treatment, however, the CBT+MT group had less depression and neuropsychiatric symptoms at the 1-year follow-up (p<0.38). No differences were found in quality of life between groups at any time point. Findings suggest that enhancing memory training with add-on interventions directly targeting negative expectations may increase their efficacy. Specifically, these add-on treatments may be effective by directly increasing elements of self-efficacy, leading to both psychological and neuropsychological benefits." @default.
