- W2897916052 abstract "Although widely used, conventional structural brain MRI detects clear volume changes in Alzheimer's disease (AD) at the symptomatic stage that correlate to widespread advanced pathologic findings. Alternative metabolic MRI techniques are now increasingly available on clinical 3T MRI scanners, and sodium 23Na-MRI has been shown to detect differences in hippocampal signal intensity(SI) between mild AD and aged matched controls in a small preliminary study with corresponding hippocampal group volume differences. In this study, we compared 23-Na-SI and volume differences in several Alzheimer's associated brain target regions between cognitively normal and mild cognitive impairment (MCI) older adults. Sixty older adults (76±6years;60% female, 30% APOE ε4 carriers, 43 cognitively normal/17 MCI [15 amnestic]) underwent 3T MRI (Trio, Siemens). 23Na-MRI was acquired with FlexTPI sequence using dual-tuned 1H/23Na birdcage head coil (Rapid Biomedical) at nominal 5mm isotropic resolution (TR=160ms, TE=0.3ms FA=90deg). 23Na-images were B0B1 corrected. 23Na images were co-registered with MPRAGE/T2 images and resampled to 1mm, output was checked by neuroradiologist. SI from 23Na images were extracted for target regions after partial volume correction. The target SI values were normalized by selected reference ROI (combined pre- and post-central gyrus). MPRAGE acquired at 1mm isotropic was segmented and parcellated with Freesurfer6.0. The volumetric measures were normalized by intracranial volume (ICV). Statistics were performed on SPSSv24 controlling for age, gender and APOE ε4 status. Whilst normalized 23-Na-SI for all target regions was significantly different between the MCI and cognitively normal groups (p<0.005), there was no significant association between age, gender and APOE e4 status on 23-Na-SI (Table1). In contrast, there was no significant group difference for the volumetric measures of the same target regions, but significant age association in 5/6 target regions and significant gender association in 2/6 target regions, with hippocampal and parahippocampal gyral volumes associated with both age and gender (Table1). Not only was 23Na-MRI able to significantly differentiate between MCI and normal older adults when there was no corresponding regional volume difference, 23Na-SI was also not shown to be associated with age and gender. 23Na-MRI shows promise in evaluation of early AD." @default.
