• W2897917963 abstract "Neuropsychiatric symptoms (NPS) are common in Alzheimer's disease (AD) and have significant prognostic implications. Apolipoprotein E-e4 (APOE e4) has been hypothesized to influence clinical expression of AD, including NPS. A systematic review and meta-analysis was performed to evaluate the association between APOE genotype and affective NPS in cognitively impaired subjects. Databases CINAHL, Embase, PsychINFO and PubMed were searched up to October 2017. Studies evaluating APOE genotype with presence of NPS (agitation, depression, anxiety, apathy, irritability, sleep problems) in cognitively impaired subjects (mild cognitive impairment (MCI), AD) were included. Study quality was assessed with the Newcastle-Ottowa Scale (NOS). Meta-analyses on pooled odds ratios (OR) using random-effects models were performed. NPS were analyzed separately. Additionally, effect of possible sources of heterogeneity was evaluated. Of the 53 included studies for meta-analysis (n = 11180), 7 studies were population based. 51.4% of the subjects were carrier of at least one APOE e4 allele. The association between APOE genotype and agitation was examined in 12 studies, with depression in 31, with anxiety in 11, with apathy in 9, with irritability in 8 and with sleep in 7 studies. Meta-analysis showed no association between APOE genotype and any of the investigated NPS. However, substantial between-study heterogeneity (I2 > 60%) was observed for outcomes depression, anxiety and irritability. Study setting (population vs. clinical), outcome assessment methods (self vs. proxy), in case of depression symptomatology vs. disorder, mean age, mean years of education and percentage females could not be identified as source of heterogeneity. There is no evidence that APOE e4 genotype is associated with the presence of agitation, depression, anxiety, apathy, irritability or sleep problems in cognitively impaired subjects. Dichotomous vs. dose effect (i.e. number of APOE e4 alleles) approaches is currently being investigated. To the best of our knowledge, this is the first study that meta-analyses the effect of APOE genotype on the most common NPS in cognitively impaired subjects, after two decades of literature reporting conflicting results. The current work highlights the need to explore other factors than APOE genotype in explaining NPS in this population." @default.
