- W2897936904 abstract "We aimed to assess stroke rates after carotid revascularization surgery and to identify factors influencing stroke. The primary end point was the incidence of 30-day periprocedural major stroke and the time to stroke postoperatively. Secondary end points included minor stroke, myocardial infarction, and overall survival. From 2003 to 2017, all patients who underwent carotid surgery at our tertiary vascular center were analyzed. The Society for Vascular Surgery carotid reporting standards were used as a guideline to assess patients’ demographics, risk factors, clinical presentation, and periprocedural details. Of 9585 patients referred with carotid disease, 690 carotid interventions were performed (carotid endarterectomy in 633 patients and carotid angioplasty and stenting in 57 patients). A major ipsilateral stroke occurred in 0.9% of patients (n = 6) within the 30-day periprocedural period. Two major ipsilateral strokes occurred at 35 weeks and 129 weeks. Stroke-free survival at 10 years was 98.9% in carotid endarterectomy and 98.1% in carotid angioplasty and stenting (P = .590). Female sex (P = .036), renal failure (P = .007), postoperative systolic blood pressure >180 mm Hg (P = .004), and renal complications (P = .019) were found to be independent predictors of stroke. After development of a major ipsilateral stroke, one patient died 4 weeks later, two patients had major motor deficits, and three patients had mild motor deficits that resolved completely. Degree of ipsilateral stenosis, plaque type, and timing of the procedure after onset of symptoms did not have an impact on the risk of postoperative stroke development. Female sex, renal failure, high postoperative blood pressure, and renal complications are independent predictors of stroke after carotid surgical interventions." @default.
