- W2897948138 abstract "We given nutrition support treatment according to the changes by nutrition status in head and neck cancer patients at the process of definitive concurrent chemoradiotherapy(CCRT). Analyze the relationship between nutrition support therapy and CCRT side effects, quality of life score, treatment effect, prognosis and other indicators. A total of 100 patients with head and neck cancer undergoing definitive chemoradiotherapy were enrolled. They were randomly divided into a nutritional intervention group (n=50) and a conventional diet group (n=50). The nutritional intervention group treated with prophylactic enteral nutrition powder formula (645.43 kcal/day; 2701.3 kJ/day) feeding before the initiation of CCRT. All patients were subjected to a standardised follow-up programme which included prospective evaluation of psychological status and HRQOL, prior to, during and after CCRT. The psychological status of those two groups were evaluated by Patient Health Questionnaire 9(PHQ-9), Generalised Anxiety Disorder Assessment 7(GAD-7) and Distress Thermometer. The primary endpoint was the nutritional indicators including body weight, white blood cells, lymphocyte, albumin etc. The secondary endpoints were the psychological status.This study is ongoing; data for this interim analysis were collected at December 30, 2017. This trial is registered with www.chictr.org.cn, number ChiCTR-IIR-17011243. Between March, 2017, and September, 2017, 60 patients were enrolled. The most common site of primary tumors was the nasopharynx (86.7%), followed by the oropharynx (8.3%), the hypopharynx (3.3%), and the larynx (1.7%). At the time of analysis, the median follow-up was 3 months for 60 patients. Both groups of patients showed different degrees of psychological changes, weight loss, bone marrow suppression and decreasing nutritional indicators in the process of CCRT. The psychological changes(P=0.021), weight loss (P=0.011), reduction of white blood cells (P=0.019), lymphocytes (P=0.001), hemoglobin (P=0.023) and the incidence of grade 3 (bone marrow suppression, salivary gland, oral mucosa and skin) of the nutritional intervention group were lower than the conventional diet group. Moreover, in the nutritional intervention group, the length of hospital stay (PLOS) was significantly shorter than the conventional diet group. While of the 60 patients, 57/60 (95%) achieved complete response (CR) of primary lesions, 3/60 (5%) achieved partial response (PR),and they did not show significant differences between groups. The present interim analysis shows that nutrition support therapy can improve the psychological status and reduce the incidence of malnutrition and normal tissue complication probability in head and neck cancer patients during CCRT. Longer follow-up is needed to determine the significance of these associations with prognosis." @default.
