- W2897957424 abstract "Clinical displays of heart pathology on the background of gallbladder (GB) abnormalities are non-specific. Their prevalence and association patterns are still not clear, which determines relevance of this research. Investigation aim was to establish the clinical peculiarities of ischemic heart disease (IHD) course depending on GB condition in patients who underwent coronary bypass surgery (CBS). We analyzed peculiarities of clinical course of IHD in 95 patients (40 % withacute myocardial infarction (AMI) and 60 % with angina pectoris (AP)), who underwent CBS. Investigation group included 21 % of women and 79 % of men, mean aged – 61,9±1,8 years. GB was intact in 30,5 % patients, biliary sludge and bent GB body were revealed in 13,7 % each, deformations of GB neck – in 27,4 %, cholelithiasis – in 10,5 %, postcholecystectomy condition – in 4,2 %. We revealed some peculiarities of IHD course in patients after CBS depending on GB condition. In subgroup with biliary sludge there were more women and fewer men than in intact GB subgroup, equal share of AMI and AP, minimal incidence of arterial hypertension (92,3 %), but maximal incidence of obesity (92,3 %) and the most frequent decrease of left ventricle systolic function comparing to other groups (69,2 %, all р<0,05). Participants with bent GB included only men with concomitant arterial hypertension. In this group percentage share of AP was higher than AMI and extrasystoles incidence was one of the highest (30,8 %). Subgroup with GB neck deformations was the most numerous (27,4 %). In this subgroup CBS was significantly more frequently caused by AP than AMI and postinfarction cardiosclerosis was revealed the most often. This subgroup included more women and fewer men than intact GB and bent GB subgroups; all patients had concomitant arterial hypertension. Subgroup with GB neck deformation also was characterized by the highest incidence of diabetes mellitus (23,1 %) and extrasystoles (30,8 %). Patients with cholelithiasis were significantly older and included the biggest share of women (40,0 %). In this subgroup CBS was significantly more often caused by AP (the most often among all groups) than AMI, all patients had arterial hypertension, but incidence of obesity was minimal (40,0 %). These patients significantly more often had atrial fibrillation (50,0 %), including paroxysmal type (40,0 %). Investigation of structural-functional heart condition and metabolic background depending on GB condition and methods of pharmacological correction is a promising field for future researches." @default.
