- W2897957567 abstract "Modifying diet may protect against cognitive decline, yet the effect of longer-term diet on cognitive function in middle-age is largely unknown. This study evaluated associations between three diet patterns during adulthood (characterized by Mediterranean diet (MedDiet), Dietary Approaches to Stop Hypertension diet (DASH) and A Priori Diet Quality Score (APDQS) diet scores) and midlife cognitive performance. We studied 2,621 CARDIA participants: 45% black, 57% female, and age 17-34 years at baseline (1985-6) who were followed for 30 years. Mean dietary pattern scores in adulthood were calculated from repeated diet history administered at years 0, 7 and 20. We evaluated 5-year change in midlife processing speed (Digit Symbol Substitution; negative z score reflects decline from baseline) and executive function (Stroop interference; positive z score reflects decline from baseline) from year 25 to 30 and global cognitive function (Montreal Cognitive Assessment (MoCA)) at year 30. Linear models adjusted for demographics, body mass index, smoking, physical activity, diabetes and energy intake were used to examine associations between tertiles of dietary pattern scores and cognitive function. MedDiet score significantly correlated with both APDQS (r=0.69) and DASH (r=0.59) scores. In adjusted models, higher DASH and APDQS scores were associated with less decline in processing speed (DASH: low=-0.05, mid=-0.00, high=0.06 and APDQS: low=-0.07, mid=-0.01, high=0.07, both PTrend=0.02) while higher MedDiet and APDQS scores were associated with less decline in executive function (MedDiet: low=0.10, mid=-0.06, high=-0.03 and APDQS: low=0.11, mid=0.01, high=-0.10, both PTrend<0.01). Participants with greatest adherence to MedDiet and APDQS, but not DASH, were significantly less likely to have poor global cognitive function (≥1 SD below mean MoCA). In adjusted models, the odds ratios (95% confidence interval) comparing extreme tertiles of the three dietary scores were 0.45 (0.33-0.62) for MedDiet, 0.38 (0.26-0.54) for APDQS and 0.88 (0.68-1.15) for DASH. Greater adherence to high quality dietary patterns during adulthood was associated with less decline in midlife cognitive function. Additional studies are needed to define the combination of foods and nutrients for optimal brain health and prevention of cognitive decline across the life-course." @default.
