Our meetings boast international attendance with international travel scholarships to encourage attendance from all parts of the world. Our journals are read widely and benefit from a discussion that is beyond an individual, program, or country's experience. The issue of gender in neurosurgery, including recruitment, retention, promotion, and leadership, has yet to be formally studied on a global international scale. While only a snapshot of a single department, the commentary and figure by Lauretti1 underscore at issue of gender in neurosurgery is worth considering on an international level. In the United States we are encouraged by increasing numbers of females matched each year, now surpassing for 3 consecutive years the theoretical critical mass of 15% to establish a presence within neurosurgery training. We maintain that mentorship will be a key component of retention and career development in both genders, but that mindfulness of potential talent is imperative. The “Rooney Rule,” adopted by the US National Football League in 2003, sets such an example. Teams are required to interview at least one minority for head coaching jobs. This does not mean that they must hire that individual; hiring should be based solely on merit. However, it does mean that a previously overlooked talent pool was now considered. Within just 3 yr, the number of minority head coaches rose from 6% to 22%.
