- W2898710005 abstract "Objectives: With renal denervation done for severe hypertension, testing for denervation is usually absent. We report preliminary observations from the AUSHAM Study (Clinical Trials.gov NCT02444442) on the feasibility of measurement of passage of the specific sympathetic nerve protein tyrosine hydroxylase (TH) to urine, from sympathetic nerves degraded by radiofrequency energy, as a test for achieved denervation. Methods: In 5 patients with resistant hypertension, a bilateral renal denervation procedure was performed with the Medtronic SPYRAL radiofrequency ablation catheter. Results are available also from one blinded sham procedure. Urine samples (20 ml) from multiple collection points after baseline, from 3 hours to 12 months post-RDN, were concentrated with ProteoSpin Urine Concentration Maxi Kit (Norgen Biotek Corp), then Western blots were run for TH, with urine creatinine concentration serving to normalize for differences in urine concentration. Results: All 5 RDN patients showed a similar pattern of TH excretion in urine. A low but detectable baseline level of TH was present, with abrupt augmentation of urinary TH in all patients (mean 369 % increase) 3 hours post-procedure. At no other time point, from one week out to 12 months, was urinary TH consistently above baseline (Figure). In the single sham patient available to-date urine TH was at low, essentially unchanging levels throughout. Conclusion: As hypothesized, TH excretion in urine was markedly augmented following a renal denervation procedure. Peak excretion was surprisingly early, evident 3 hours post-procedure, and only variably present at one week. The present observation, if confirmed in the ongoing AUSHAM study, could be used to retrospectively document achieved renal denervation." @default.
