- W2899181508 abstract "Objectives: To explore the relationship of different blood pressure (BP) parameters with target organ damages (TODs) and to determine the optimal BP parameters with a high accuracy of prediction. Methods: We assessed the left ventricular mass index (LVMI) and carotid intima-media thickness (CIMT) using ultrasonography and microalbuminuria (MAU) as measures of TODs in 266 hypertensive patients without previous treatments. BP were collected using the office BP, home BP and 24 h ambulatory BP, and the BPV was evaluated using coefficient of variation (CV). Results: In the univariable and multivariable linear regression analysis, ambulatory SBP and mean home SBP were always significantly correlated with LVMI, CIMT and MAU (all P < 0.05); However, there was no independent association with all three TOD indices in terms of DBP, mean office SBP and maximum SBP, and long-term BP variability (BPV) parameters (all P > 0.05); Short-term SBPV and pulse pressure (PP) were not always correlated with all three TODs indices. Multivariate logistic regression analysis showed that 24 h SBP, day-time SBP, night-time SBP and mean home SBP were still the top four BP parameters independently and significantly associated with left ventricle hypertrophy (LVH), carotid atherosclerosis (CAS) and albuminuria (all P < 0.05). Receiver operating characteristic (ROC) curves showed that ambulatory SBP and mean home SBP had a high predictive value for TODs. Conclusion: Ambulatory SBP and mean home SBP may be the most important and optimal BP parameters for accurate prediction of TODs. In clinical applications, they can be used as the preliminary screening indicators for predicting TODs in hypertensive patients." @default.
