- W2902149920 abstract "Abstract Brachial plexus avulsion (BPA) is common in polytrauma victims, and the avulsion of preganglionic dorsal nerve roots can cause severe and nonremitting chronic pain. Dorsal root entry zone lesioning (DREZotomy) is a safe and effective treatment for pain resulting from BPA. Over the past 40 years, centers around the world have reported successful and durable treatment of pain in two-thirds of patients undergoing the procedure. With refinement of microsurgical techniques and the availability of intraoperative neuromonitoring, success rates have improved and disabling complications are rare. In this chapter, we discuss the anatomical and physiological considerations in the DREZotomy and the role of magnetic resonance imaging in patient selection. A minimally invasive cervical spine approach and microsurgical techniques are presented. The history of DREZotomy and published series on efficacy and complications are reviewed." @default.
