- W2904697675 abstract "This chapter describes definition, etiology, pathology, diagnosis, treatment and management of Paget disease of bone. This is the second most common bone disease after osteoporosis. Both genetic and environmental factors have been implicated in the pathogenesis of Paget disease. Paget disease occurs commonly in families and can be transmitted vertically in an autosomal dominant pattern. Pagetic bone contains more numerous osteoclasts than normal, and these osteoclasts contain substantially more nuclei than normal osteoclasts. When Paget disease is suspected, the diagnostic evaluation should include a careful medical history, including family history of the condition and symptom history, and a focused physical examination. Laboratory tests include measurement of serum total alkaline phosphatase (SAP), and serum calcium and 25-hydroxyvitamin D if bisphosphonate treatment is considered. Current guidelines suggest treatment of most patients with active disease at risk for complications with high potency bisphosphonates, either intravenous zolendronic acid, or oral alendronate or risedronate." @default.
