- W2906156737 abstract "The main objective of this study was to identify the factors associated with readmissions in heart failure patients. A retrospective observational study was conducted in a tertiary care teaching hospital of Southern India. Institutional ethics committee approval was obtained prior to the study. Patients admitted with heart failure (HF) between January 2012 to December 2016 were identified from MRD patient files using ICD 10 code 150. Patients information on demographics, medical history, laboratory parameters, drug treatment, number of readmissions were retrieved from the patient medical records. Factors associated with readmissions were identified by multiple logistic regression and odds ratio (OR) was calculated. A total of 451 patients were included in the study. Mean age of the study population was 59.7±14.1 consisting of predominantly male patients (63.6%). Mean duration of hospitalization was 7.6±4.8 days and 35.2% (n=159) of patients were readmitted more than once. Anemia (50.4%), diabetes mellitus (44.7%), hypertension (42.4%) were the most common comorbidities present in the study population. Hypothyroidism was present in 4.2% (n=19). Diuretics (90.6%) digoxin (44.1%) and ACE inhibitors (32.6%) were the most commonly used drugs among study population and highest clinical improvement was seen in patients treated with diuretics (90.6%), followed by digoxin (43.8%) and ACE inhibitors (28.1%). Diabetes mellitus (OR:1.59; 95% CI: 1.08-2.35; p<0.019), hypertension (OR:1.53; 95% CI:1.03-2.25; p<0.033) and hypothyroidism (OR: 2.64; 95% CI:1.04-6.7; p<0.035) were identified as statistically significant factors associated with increased readmissions in HF patients. Use of ACE inhibitor had statistically significant effect on reduction of more than one readmissions in HF patients (OR:0.54; 95% CI:0.29-0.99; p<0.044). Comorbidities like diabetes mellitus, hypertension and hypothyroidism were found to increase the readmissions in HF patients in this study. ACEI therapy appears to decrease the chances of additional readmissions." @default.
