- W2906621787 abstract "In cost-utility analysis (CUA), main result is the ICER where effectiveness is expressed in QALY. Understanding utility valuation is important to assess ICER validity. The objective of this study was to describe how utility is implemented in oncology efficiency opinions (EO) and how it was evaluated by the National Authority of Health (HAS). All the EO related to oncology drugs since 12/2014 by the HAS were reviewed. We collected utility values implemented in CUA, their data sources, as well as the methodological reservations related to utilities that were assessed by the HAS, by severity grade (minor/important/major). 21 oncology EO were identified (14 in solid tumors and 7 in hematology). Over 188 reservations raised by HAS, 28 (15%) were related to utilities. They were classified as minor in 20 cases (71%), important in 8 cases (29%) and none was major. Reservations were mostly associated to data validity (50%), to disutility methods applied in CUA (29%). Less frequently, reservations are the use of non-representative data of the French population (11%) and of different adverse events valuation for cost and disutility (11%). Regarding reservations for data validity, the HAS underlined the heterogeneity of utilities sources. Indeed, post-progression utilities were collected from clinical trials (CT) in 54% of cases and from literature review (LR) in 46% of cases. For the experimental arm an average decrease of 33.8% for CUA using LR and of 14.3% for CUA using CT was observed between utility scores of progression-free (PF) and progressed disease (PD) health states (respectively 18.4% and 9.8% for the controlled arm). This analysis is the first to evaluate utility data in French EO. Most utility reservations were related to data validity and none was major. In the future, the use of utility data from the same source for PF and PD health states is recommended." @default.
