- W2910368736 abstract "The intensification of neoadjuvant therapies (i.e., chemoradiotherapy [CTRT] before surgery has provided a shift in the paradigm of treatment of locally advanced rectal cancer [ [1] Sauer R. Becker H. Hohenberger W. et al. Preoperative versus postoperative chemoradiotherapy for rectal cancer. N Engl J Med. 2004; 351: 1731-1740https://doi.org/10.1056/NEJMoa040694 Crossref PubMed Scopus (4703) Google Scholar ]. More recently, despite the short follow-up of published studies [ [2] Cercek A. Roxburgh C.S.D. Strombom P. et al. Adoption of total neoadjuvant therapy for locally advanced rectal cancer. JAMA Oncol. 2018; 4https://doi.org/10.1001/jamaoncol.2018.0071 Crossref Scopus (273) Google Scholar ], the preoperative addition of full doses of induction systemic chemotherapy before CTRT is emerging as a potential new standard of care. Comment on ‘Adjuvant chemotherapy in patients with rectal cancer achieving pathologic complete response after neoadjuvant chemoradiation and surgery’European Journal of CancerVol. 112PreviewWe read with great interest the letter published in the European Journal of Cancer by Tomasello et al. assessing the issue of whether adjuvant chemotherapy is necessary for patients with locally advanced rectal cancer who have achieved pathologic complete response (pCR) [1]. In their letter, Tomasello et al conducted a meta-analysis, and their results indicated that patients with rectal cancer achieving pCR after neoadjuvant chemoradiation should receive 5-fluorouracil–based adjuvant chemotherapy as an improved overall survival (OS) could be observed in the adjuvant chemotherapy group compared with the non-adjuvant chemotherapy group (HR = 0.50, 95% CI = 0.43–0.59, P < 0.01).
