- W2912064076 abstract "Background Long-term exposure to air pollution has been linked to increased mortality and morbidity risk. We used the Myocardial Ischaemia National Audit Project (MINAP) data of patients admitted to hospital with acute coronary syndrome (ACS) to determine whether long-term exposure to air pollution was associated with all-cause mortality or hospital readmission for ACS in these patients. Methods Patients from MINAP data who were admitted to hospital for ACS and resident in the greater London at the time of admission (between 1 Jan 2003 and 31 Mar 2007) were followed-up for all-cause mortality and readmissions for ACS until the date of readmission, death, or 28 Feb 2010. Annual average concentrations of NO2, NOx, O3, PM10, PM2.5, PM10-2.5 and traffic exhaust and non-exhaust contributions of PM10 and PM2.5 for the years 2003 to 2010 were from a dispersion model with 20×20m resolution. Individual level exposure was defined as the average concentration at model grid points within 25m of each patient's postcode centroid. Multiple imputation was used to impute missing values for covariates (including time, age, reperfusion, deprivation, smoking, ethnicity, diabetes, angina, previous MI, and medication). Hazard ratios (HR, 95% CI) were estimated using a Cox proportional hazards model fit to each imputed dataset and combined to get an overall HR. Results Long-term exposure to most air pollutants was positively associated with death or readmission for ACS (number of events=5378) in single pollutant models. We observed a HR of 1.06 (95% CI 1.00-1.11) per IQR (7.9 µg/m3) increase in NO2 and of 1.08 (1.00-1.16) per IQR (1.5 µg/m3) increase in PM2.5 after adjusting for covariates. O3 was highly inversely correlated with NO2 and PM2.5; the association for O3 was protective (HR 0.94, 0.88-1.00). Conclusions These findings suggest that among ACS survivors, long-term exposure to nitrogen oxides and particulate matter increases the risk of all-cause mortality or re-admission to ACS." @default.
