- W2912154094 abstract "The recent article by Palloshi et al 1 Palloshi A. Fragasso G. Silipigni C. Locatelli M. Cristell N. Pala M.G. Alfieri O. Margonato A. Early detection by the Tei index of carvedilol-induced improved left ventricular function in patients with heart failure. Am J Cardiol. 2004; 94: 1456-1459 Abstract Full Text Full Text PDF PubMed Scopus (17) Google Scholar has major conceptual flaws regarding the use of the Tei index based on 2 false assumptions made by the authors. They wrote that the Tei index is independent of (1) heart rate (HR) and (2) loading conditions. This is not true, or, at best, controversial. Only 2 articles have reported on the Tei index/HR relation: in the first landmark article by Chuwa Tei 2 Tei C. Ling L.H. Hodge D.O. Bailey K.R. Oh J.K. Rodeheffer R.J. Tajik A.J. Seward J.B. New index of combined systolic and diastolic myocardial performance: a simple and reproducible measure of cardiac function: a study in normals and dilated cardiomyopathy. J Cardiol. 1995; 26: 357-366 PubMed Google Scholar (not specifically designed to address this relation) it was said “the value of the index did not appear to be related to heart rate,” whereas in the second 3 Poulsen S.H. Nielsen J.C. Andersen H.R. The influence of heart rate on the Doppler-derived myocardial performance index. J Am Soc Echocardiogr. 2000; 13: 379-384 Abstract Full Text Full Text PDF PubMed Scopus (115) Google Scholar (specifically addressing this relation using incremental atrial pacing), the Tei-index was influenced by HR, although it was a weak correlation (the Tei index increased on average 0.02 ± 0.03 per 10 beats/min increase in rate). This obviously cannot be presented to readers as “it is not affected by heart rate …” as firmly stated in the article by Palloshi et al. 1 Palloshi A. Fragasso G. Silipigni C. Locatelli M. Cristell N. Pala M.G. Alfieri O. Margonato A. Early detection by the Tei index of carvedilol-induced improved left ventricular function in patients with heart failure. Am J Cardiol. 2004; 94: 1456-1459 Abstract Full Text Full Text PDF PubMed Scopus (17) Google Scholar This point is not trivial in this study, because carvedilol potently lowers HR. In the study, HR was lower after carvedilol, even if there was no statistically significant difference (however; the very small sample may explain lack of statistical difference). This lower HR could partly explain the difference in the Tei index; at least some caution and discussion should have been posed on this point. ReplyAmerican Journal of CardiologyVol. 96Issue 3PreviewThe Tei index is a relatively new echocardiographic/Doppler index of combined systolic and diastolic function. Recently, increasing and well-grounded evidence has been promoting the Tei index as a good diagnostic as well as prognostic tool in patients with heart failure. Feasibility and less dependency on the operator, ventricle geometry, heart rate (HR), preload, and afterload represent the best suggested characteristics of this index.1,2 Very few studies have evaluated the possible dependency of the Tei index on HR and hemodynamic status.
