- W2912171314 abstract "Controversy surrounds the relation between gastric surgery and gastric cancer. Is gastric surgery a causative factor in the carcinogenic process or is the association purely coincidental? Historically, carcinoma of the gastric stump is most closely associated with simple gastroenterostomy and Billroth II gastrectomy. An 84-year old man who had distal gastrectomy with Billroth II gastrojejunostomy at age 19 for peptic ulcer disease (PUD) developed mild iron deficiency anemia in early 2015. Upper and lower endoscopy revealed no source of bleeding. The gastrojejunostomy site appeared normal. In 2016, he developed epigastric pain and worsening anemia. An EGD with biopsy in September 2016 revealed a large ulcerated lesion proximal to the gastrojejunostomy anastomosis. Histology showed moderate to poorly differentiated adenocarcinoma with locally advanced disease and hepatic invasion. He had subtotal gastrectomy with Roux-en-Y gastrojejunostomy and partial hepatic resection. Immunohistochemistry (IHC) positive for CK7, CDX2, and CK20. HER2neu (-) by IHC & FISH. Past medical history includes prostate cancer s/p prostatectomy, remote heavy alcohol use >40 years ago, (-) H. pylori. Family history is remarkable for female breast cancer. Bile acid reflux and achlorhydria are postulated as mechanisms for gastric stump carcinoma. A European cohort of >2,600 postgastrectomy patients followed for 4 decades suggests that gastric surgery does predispose to gastric cancer 15-20 years postoperatively. If gastric surgery predisposes to gastric stump cancer with 2-4 fold risk after 20 years, what are the implications for clinical practice? Are all longstanding postgastrectomy patients at increased risk of stump cancer or is risk related to host factors? There is a cohort of patients who underwent Billroth II gastrectomy for management of ulcers before the advent of PPIs. These patients may be at risk for primary gastric cancer. No formal guidelines exist in the U.S. for endoscopic surveillance after gastrectomy performed for ulcers.
