- W2912271481 abstract "Background: Leukaemia is the most common cancer in childhood. The aetiology of leukaemia is largely unknown. Several hypotheses involve environmental exposures and may implicate spatial clustering of cases. Results from previous studies on spatial clustering of childhood leukaemia were heterogeneous, though the majority of them suggest that cases do cluster. Most previous studies have used regional count data and were thus susceptible to the modifiable area unit problem and to unstable rates. We investigated whether there is spatial clustering of childhood leukaemia in Switzerland using exact geocodes of residence and methods applicable to point pattern data. Methods: We included 1815 leukaemia cases diagnosed at age 0-15 years during 1985-2015 from the Swiss Childood Cancer Registry. Age and sex matched controls (10 per case) were randomly sampled from the national census (1990, 2000, 2010) closest to the time of diagnosis of each case. Geocodes of place of residence were available at time of diagnosis for cases and at census for controls. We computed the difference in k-functions, Cuzick-Edward’s test and Tango’s index for point data to assess spatial clustering and Kulldorff’s circular scan to detect individual clusters. We used the minimum profile p-value method to adjust for multiple testing. Results: Differences in k-functions between cases and controls suggested stronger clustering among cases for distances of 200-400 m (overall p=.06). Cuzick-Edward’s test showed evidence for spatial clustering of cases when considering the 2 nearest neighbors (adjusted p=.008), while Tango’s index showed evidence of clustering when using a theta parameter of 30 km (adjusted p=.007). No evidence for individual clusters was found by Kulldorff’s circular scan. Conclusions: The findings suggest that childhood leukaemia tends to cluster in space and environmental exposures may play an important role in the aetiology." @default.
