- W2912284311 abstract "Medical studies often require a randomized, parallel or crossover design, and comparison of any active treatment with a placebo one. This is especially actual when the outcome assessment is based on subjective parameters as is typically the case in studies on antidepressant or analgesic effects of rTMS. The ideal placebo rTMS should fulfill a number of criteria: (i) the position of the active and placebo coils over the scalp should be the same; (ii) the subjective somatic scalp sensation and the acoustic artifacts during stimulation should also be the same for active and sham coils, but (iii) no physiological effect on the targeted cortical region should occur for the placebo stimulation. Sham and real TMS with synchronous sound masking and skin sensation masking with electrical stimulation were performed in 9 healthy male with a mean age of 34.2 years (SD = 6.7 years). Neuro-MS/D magnetic stimulator (Neurosoft, Russia) with Neuro-MS.NET software is used for testing HF rTMS of the left DLPFC (MDD treatment protocol). The synchronous sound masking was performed using a monitor speaker connected to magnetic stimulator via trigger unit and located near discharging coil. The synchronous 10-Hz electrical stimulation was given through the electrodes fixed on the head along the front edge of the coil to mimic the sensation. The electrical stimulation amplitude for TMS-naïve subjects was from 2 to 5 mA. A procedure of titration of the electrical stimulation amplitude was previously performed for non- naïve subjects. It allowed to choose individually such stimulation amplitude, which on the one hand, is acceptable for subjective sensations, and on the other hand, does not allow to distinguish whether there is TMS or not. The tested solution for sham TMS (software Neuro-MS.NET software ) allows to achieve realistic sham TMS at 100% of TMS-naïve and 88.9% of non-naïve volunteers." @default.
