- W2912286959 abstract "Objective: To determine whether individuals with Parkinsonism can improve their walking pattern via instructions to amplify their arm swing excursions and to determine if enhanced arm swing improves or degrades gait performance. Background: The parkinsonian gait is narrow based with poor or no arm swing. There may be difficulty initiating gait, hesitation, freezing, and difficulties maintaining cadence. Arm swing stabilizes the body’s angular motion about the vertical axis of the foot. In the absence of arm swing there is greater reaction moment of the foot. This creates largely unopposed force generated by the leg and torso. Additionally, arm swing decreases torque placed on joints and skeletal muscle. Design/Methods Neurologic examination and UPDRS (Parts II and III), and Montreal Cognitive Assessment Tool. Participants walk along a 22′ GAITrite walkway measuring footfall patterns and Functional Ambulatory Profile (analysis of fall risk). All conditions: Arm swing is quantified using infrared videography via a reflective arm band. Angular motion can be detected in the X, Y, and Z plane via VICON motion capture. Condition 1: Walking only (normal pace with no additional instructions) Condition 2: Walking with one variable (verbal swing instruction) Results: The change between the mean arm swing angle at baseline compared to instructed is 6.95 degrees. The mean functional ambulatory profile score at baseline was 91.34. The mean functional ambulatory profile with instruction was 92.22. Conclusions: The authors believe that the increase in functional ambulatory profile from condition 1 to condition 2 can be attributed to arm swing. First, the degree of arm swing was the only change between condition 1 and 2. Second, the change in arm swing angle was statistically significant. Conclusively, the degree of arm swing increased with statistical significance and resulted in a higher FAP, which correlates with a reduction in falls risk.
