• W2912351211 abstract "Objectives: Subject-level meta-analyses assessing the age-specific relevance of blood pressure (BP) to fatal and non-fatal events have been published for office BP, but not for out-of-the-office BP, the guideline-recommended way of BP assessment. Methods: Fatal and non-fatal outcomes were collected in 17,383 participants from 17 population cohorts. At baseline, office BP and either ambulatory (n = 12,086) or home (n = 6887) BP were assessed. Using multivariable Cox regression, risk was computed across four age groups (≤60, 61–70, 71–80 and > 80 years) and five categories of systolic and diastolic BP. Results: Over 231,054 person-years, 3794 people died, 1415 of cardiovascular disease, and 2914, 1294 and 1168 fatal or non-fatal cardiovascular, coronary or cerebrovascular endpoints occurred. Irrespective of the type of BP measurement, in all age groups and for all outcomes, there was a graded increase in risk with higher systolic/diastolic BP starting from levels below 110/65 mm Hg. Although absolute risk increased with age, the hazard ratios expressing relative risk associated with in-office and out-of-the-office BP showed an opposite trend, generally increasing (P ≤ 0.0365) from the oldest to youngest age group. These age-related trends in relative risk showed great consistency in both sexes (51.1% women) and in 13,942 Europeans and 3441 Asians. Risk functions, expressing the 10 year incidence of endpoints, were steeper for out-of-the-office than in-office BP, nighttime ambulatory BP being the strongest predictor. Conclusion: Our findings highlight the necessity to generalise out-of-the-office BP measurement, as the standard in hypertension management and the need for timely hypertension management in middle-aged adults." @default.
