- W2912389182 abstract "Introduction: Baby boomers (individuals born from 1945-1965) are five times more likely to have Hepatitis C virus (HCV) than other adults. HCV screening is recommended for all baby boomers to reduce HCV-related morbidity and mortality. The Veteran Affairs (VA) system is the largest healthcare provider to those who have HCV. HCV screening may be missed during routine patient encounters. Hence, there is a need to identify unscreened baby boomer veterans. The aim of our study was to develop a specific patient-centered tool to improve screening for HCV in this population. Methods: Using a VA database, 3,194 unscreened baby boomer veterans linked with primary care within our Veterans Integrated Service Network (VISN) were identified. Patients with short life expectancy or who moved outside the VISN were excluded after chart review. A standardized letter was developed addressing the target population (Image 1). Letters were sent using certified mail to the veterans and HCV antibody serology testing was ordered for each patient. Results were received as a view alert to one of the providers. Negative test results were communicated via letters. Positive test results were discussed with patients via phone. For patients with positive results, an appointment was scheduled in the Hepatology specialty care clinic for further care. Results: From January 2017 until May 2017, 1,350 (42.3%) veterans were tested. A total of 22 (1.6%) had reactive HCV antibody. Of those who had a reactive HCV antibody, nine (41.0%) had detectable HCV RNA. Conclusion: In this cohort of veterans, compliance with HCV screening increased overall from 80.9% to 90.0% in less than 6 months with implementation of the standardized letter.
