One challenge in studying air pollution and preterm birth risk is controlling for hospital-level utilization of medical interventions that influence the timing of delivery and may vary among communities and hospitals. We assessed the relationship between air pollution and preterm birth risk using New York City (NYC) birth certificates from 2008-2010 linked to hospital records. Singleton live births with 22 to 42 completed weeks’ gestation to nonsmoking mothers residing in NYC (N=258,294) were included, and their exposures to ambient fine particles (PM2.5) and nitrogen dioxide (NO2) during the 1st and 2nd trimesters within 300 m of the maternal address were estimated using a combination of data from New York City Community Air Survey and federal regulatory monitors. We estimated the odds ratio of all and spontaneous-only preterm (gestation < 37 weeks) and very preterm (gestation < 32 weeks) births in a logistic mixed model adjusting for maternal characteristics with random intercepts for either Census Tracts of maternal residence or hospital of birth. Hospital-level rates of non-spontaneous births varied from 23% to 73% across the 42 hospitals with birth facilities in NYC and were negatively associated with mean 1st and 2nd trimester PM2.5 and NO2 exposures (?: -0.22 to -0.19). Both crude and adjusted models with random intercepts by Census Tract yielded significantly protective effects with odds ratios as low as 0.89 per 10 µg/m3 PM2.5 and 0.87 per 20 ppb NO2. Models that included random intercepts by hospital and limited the outcome to spontaneous-only preterm births yielded null associations. Spatial correlation between hospital practices and air pollution levels can confound the association between air pollution and risk of preterm birth." @default.
