- W2912408715 abstract "Introduction: Endoscopic mucosal resection (EMR) is a minimally invasive technique utilized for therapeutic resection of precancerous and early cancerous lesions throughout the GI tract. We reviewed characteristics of different colorectal lesions managed with EMR at our tertiary care center. Methods: A retrospective analysis was done on patients referred to our center between January 2013 and December 2015 for EMR of colorectal lesions. Variables such as polyp size, number, location, Paris classification, Kudo pit pattern, morphological characteristics, and histologic appearance were analyzed and reported. Results: A total of 417 patients were included in the study with mean age of 67.5±10.1 years. In 406 cases (97.2%), 1 or more >10 mm polyps were resected during a single colonoscopy. More than one polyp was found in 121 patients (28.9%). The mean size was 26.3 mm and majority of the lesions were Paris classification IIa (203; 48.7%). Reported morphology of the lesions (Table 1) was granular in most cases (107; 25.7%). Kudo pit pattern IIIs was found in 98 cases (23.5%). Majority of the lesions were found in ascending colon (135; 32.4%). On histopathology (Table 2), majority of the lesions were reported to be tubular adenomas (171; 41%) followed by tubulovillous adenomas (140; 33.6%). Residual adenoma at EMR site was found in 33 out of 417 patients (7.9%) at first surveillance colonoscopy.Table: Table. Summary of morphological characteristics of the lesionsTable: Table. Summary of histopathological characteristics of the lesionsConclusion: The utility of EMR rests in its ability to provide accurate histological nature of the superficial GI neoplasms and serves as a minimally invasive technique for removal of superficial benign and malignant lesions (Figure 1). The most common type of Colorectal lesion in our center requiring EMR was a granular, Paris classification IIa, Kudo pit pattern IIIs, tubular adenoma in ascending colon (Figure 2).Figure: The schematic representation of steps involved in Endoscopic Mucosal Resection.Figure: Intra-operative images of steps involved in Endoscopic Mucosal Resection of 30 cm, Paris Class IIa granular adenoma in Ascending Colon." @default.
